[A standard focus on form sheet.  Perhaps a little bit pedantic, but the assumption is that if I was going to do a boring  focus on form lesson, than the least I could do was make it more interactive instead of just giving the students the structure.  This should be divided into two handouts, where the *********** appear, a question sheet, and then an answer sheet to be given out later.]
