Presentation Feedback Forms for English for Academic Purposes Classes

[These are the feedback forms I'm currently using to help evaluate student presentations in my English for Academic Purposes classes.  The first one is the form I use to evaluate student presentations (Google drive, docs, pub).   I use a paper version to take notes in class during the presentation, and then later use the template to type it out into another document before giving it back to the student.  During this process, I adjust the amount of space as needed.  So for example, if I need room to make more grammar mistakes, I will just add extra boxes.  The first 6 categories (pronunciation, intonation, volume, speed, and eye contact) and their descriptors are borrowed from the evaluation feedback form published by the English for Academic Study: Speaking by Garnet Education.  Everything else is my own. 
 The second from is the one that I have the students use to give feedback to their peers (Google drive, docs, pub).]
