Graded Reader Survey From

[I normally do a lot of graded readers with my Young Learner classes on the theory that at a younger age, focusing on comprehensible input is better than grammar practice.  Up until recently I have just chosen the Graded Reader myself, but many of my colleagues give their classes a selection of choices, and allow their classes to vote on the graded reader.  I decided to try this in my class.  I chose 4 graded readers.  As a class, we read through one chapter from each book, and then the students voted on which graded reader they wanted to continue.
It was a good idea in theory, but in the morning class I found that the students did not actually have the patience to sit still and listen to 4 chapters one right after the other.  They began groaning and getting restless at the introduction of the 3rd and 4th book.  So in the afternoon class, I devised this survey form as a way to get the students talking about the books (and moving around the room) in between the chapters, instead of waiting until the end for the discussion.  I found this worked much better than the morning class.]
